String instrument rental is $100 per school year.  All Middle School and High School students who need to rent a violin, viola, cello or bass for home use during the academic year must complete the form below. Prior to completing this form, please confirm with your child's teacher that an instrument is available.

If your family is eligible for the Free and Reduced Meal program and would like to apply for an instrument rental fee waiver, please choose the Fee Waiver payment option and check the "Fee Waiver" box in this form.   When you choose this option, $0.00 will appear under price.   Eligibility will be confirmed through the district.
