2.  THE BLOCK THAT CLASS TAKES PLACE 

3.   NAME OF THE ART TEACHER FOR THAT CLASS

With this information, you will be able to complete this form and submit for processing. 

Please note that lab fees vary depending upon the class. 

Please complete a separate transaction if you have more than one student in an Art class.
